Arizona Department of Corrections, Rehabilitation and Reentry Department Order 802, effective March 2, 2022, governs inmate grievances. An inmate first files an Informal Complaint (Form 802-11) with the unit CO IV Grievance Coordinator within 10 workdays of the action complained of; the responder has 15 workdays to reply. A Formal Grievance (Form 802-1) follows within five workdays of the informal response, with a Deputy Warden-approved answer due in 15 workdays. Non-medical appeals go to the Assistant Director within five workdays (30 calendar days to respond) and then to the General Counsel, whose decision exhausts department remedies; medical appeals end with the Contract Facility Health Administrator. Sexual-abuse grievances have no filing deadline, require no informal step, trigger a written response within 48 hours where imminent risk is alleged, and get a final decision within 90 workdays. The order states the full formal process generally runs 120 calendar days.
