This is the official appendix form to Rule 32 of the Alabama Rules of Criminal Procedure, used to file a state post-conviction petition. The instructions require the petition to be legibly handwritten or typewritten and signed under penalty of perjury, limit a single petition to judgments entered in one trial, and warn that failing to include a ground for relief may bar raising it in a future petition. The original and two copies must be mailed to the clerk of the court of conviction. Listed grounds include constitutional violations such as ineffective assistance of counsel, lack of jurisdiction, a sentence exceeding the legal maximum, custody after a sentence has expired, newly discovered material facts, and a failure to appeal on time that was not the petitioner's fault. An in forma pauperis declaration with a prison account certificate is attached. The form states it cannot be used to challenge loss of good time, custody classification, or prison conditions.
