Alabama's Constituent Services intake form lets family members, friends, advocates and others raise a concern about a specific incarcerated person. It requires the submitter's relationship to the offender, the offender's name and AIS number, and the current facility selected from a list of all ADOC institutions and work centers. Concern categories include Inmate Safety, Inmate Health Concern, Medication, Disciplinary, Inmate Restrictions, Inmate Wellness Check, ADA, and PREA (Prison Rape Elimination Act); choosing Facility Concerns opens a second menu covering Extortion, Facility Conditions, Money, Property, Phone, Packages, Staff and Tablet. The form also asks whether the concern was reported before, to whom, and whether it was resolved.
